DEPOSIT -- TERMS AND CONDITIONS

Quantum Wellness Spa is offering a Founders Membership for the first 100 people that sign up for a Quantum Wellness Spa membership.  

As a Founding Member, you will receive the following benefits:

• No Initiation/Enrollment Fee – a $200 savings
• No minimum commitment, month to month
• $200 monthly membership fee – a $1200 per year savings. 
• Free access to the Rejuvenate Zone (with a max of one session, or one treatment, or one class per day). This Rejuvenate Zone free access includes the following in-spa sessions, treatments, or classes:
• group sauna
• cold plunge
• Earth room
• sound bath
• breath-work class
• meditation class
• yoga class 
• A complimentary health and wellness assessment
• Discounts on all retail and spa products
• Discounts on all spa offerings and services
• Discounts on spa day-passes and classes/treatments for friends and family
• Exclusive Founders Member events
• Founders Membership limited edition hoodie, towel, and sauna cap.
• Access to our 2 acre rooftop before and after Spa visits
• One week exclusive access to spa for Founding Members ONLY, before spa opening aka "Founder's Week"
  

The Founders Membership is limited to the first 100 people that sign up for a Quantum Wellness Spa membership.  

By signing up for the Founders Membership, and to receive the applicable Founders Membership benefits, you agree to and acknowledge the following terms and conditions:

• $20 Deposit – to be paid immediately upon signing up for the Founders Membership using an accepted credit card or bank account payment option. This will also give you access to your "Founder's Week" exclusive access before the official spa opening.
  
• The monthly membership fee ($200) will be deducted automatically, from the credit card or bank account used for the initial $20 deposit, on the formal opening day of Quantum Wellness Spa. 
• You can request the $20 refund anytime before spa opening and start of monthly membership fee, including during "Founder's Week".
• The $200 monthly membership fee will be recurring and automatically deducted/charged each month from/to the applicable and authorized credit card or bank account you place on file for the applicable Founders Membership.  You agree to execute and comply with Quantum Wellness Spa’s recurring payment authorization form and agreement to authorize the recurring monthly membership payment to be automatically deducted from/charged to the authorized payment option.
• You agree to execute and comply with the Quantum Wellness Spa Membership Agreement and all included and related terms, conditions, and obligations.
• You agree to execute and comply with the Quantum Wellness Spa liability, release, and consent forms applicable to any Quantum Wellness Spa classes, treatments, or services.
• You agree to comply with all Quantum Wellness Spa policies, procedures, and guidelines, including, but not limited to, those relating to health and safety, conduct, and spa and equipment use.
• You must provide accurate and truthful information on and in all Quantum Wellness Spa agreements, forms, consents, and any other documents you are required to execute in connection with your Quantum Wellness Spa membership or spa use.
• If you request and receive a refund of the $20 deposit, or not make the $200 initial monthly payment, on or before the formal opening day of the spa, then you will not be considered a Founding Member, and will not receive the Founding Member benefits.
  
• Violations or breaches of Quantum Wellness Spa’s Membership Agreement, payment authorization form/agreement, liability/release/consent forms, its policies, procedures, and guidelines, and/or these terms and conditions will result in the loss and discontinuation of Founders Membership benefits and/or your Quantum Wellness Spa membership.

I have read and understand the above terms and conditions regarding Quantum Wellness Spa’s Founders Membership and related benefits and agree to comply with the terms and conditions.
